Component / system
Defect
Coach and Equipment Mfg. Corp. (Coach and Equipment) is recalling certain 2025-2026 Phoenix and 2026 Metrolite buses, equipped with certain QRT-360 wheelchair retractors. The retractors may not lock, preventing the wheelchair from being properly secured.
Risk / consequence
An unsecured wheelchair can move during transit, increasing the risk of injury.
Remedy
Dealers will inspect and replace retractors as necessary, free of charge. Owner notification letters were mailed June 5, 2026. Owners may contact Coach and Equipment customer service at 1-800-724-8464.
Chronology of events
- The LOCKING of the retractor can be confirmed by assuring the locking mechanism is engaged and that the 2 webbing and hook do NOT extract out of the retractor. - Once all four (4) Wheelchair Tiedown retractors are secured to the floor anchorage and wheelchair frame, you MUST rock the wheelchair back and forth to assure proper LOCKING and tensioning of all four (4) retractors. Remove from Service. DO NOT USE! Cut the webbing near label location. 3 Either return hook and label section to Q’Straint OR provide a photo of the cut retractor(s) and wait for Q’Straint’s CUT WEBBING NEAR authorization before disposing them. LABEL LOCATION Provide the Following Information Property Name Retractor Replacement Confirm Part Number (Q011002) 4 Contact Q’Straint Customer Service (800)-987-9987 for a Date of Manufacture (AUG-25 or SEP-25) Return Material Authorization (RMA) number for Replacement. Fitting Type: PLI (-L) or Slide N Click (-SC) Total Quantity of units 1 of 1
